How do I set up MIPS claims reporting in Barti?
Quick answer: Set a Claim Type on every Medical accepted insurance in Organization Settings > General, including at least one set to MEDICARE, then turn on Enable MIPS Claims Reporting. Barti then adds Measure #141 and #226 quality codes to Medicare claims automatically.
Who this is for: Barti administrators and billing staff at small eye care practices (15 or fewer eligible clinicians) that report MIPS quality measures through Medicare Part B claims.
Before you start
You need access to Organization Settings in Barti.
Your practice must already have its Medicare payer added to Accepted Insurances. If it isn't there yet, add it first — the MIPS toggle will not turn on without it.
MIPS claims reporting is released behind a feature flag. If you don't see the MIPS Claims Quality Measures card in General Settings, contact Barti Support to have it enabled for your organization.
Register for your MIPS Value Pathway on the CMS Quality Payment Program site separately. Barti adds the quality codes to claims; it does not register you or submit anything to CMS on your behalf.
Step 1: Set a Claim Type on each Medical accepted insurance
The Claim Type field tells Barti which of your accepted insurances is Medicare. This is how Barti decides whether a given patient's claim is eligible for MIPS quality codes, so it has to be filled in before the feature will turn on.
Open the profile menu and select Organization Settings.
Select the General tab.
Find the Accepted Insurances card.
Select an insurance to edit it, or select Add Insurance to create a new one. The Edit Accepted Insurance or Add Accepted Insurance window opens.
Confirm the Insurance Type is set correctly:
Medical — medical plans. These are the rows that need a Claim Type.
Vision — vision plans. These never need a Claim Type.
For every Medical insurance, choose a value in the Claim Type dropdown:
MEDICARE
MEDICAID
TRICARE
CHAMPVA
GROUP HEALTH PLAN
FECA BLK LUNG
OTHER
Select Save (or Add for a new insurance).
Repeat for every Medical insurance in your list. At least one must be set to MEDICARE.
Important: Claim Type must be set on every Medical accepted insurance, not just the Medicare one. A single Medical row left blank will block the MIPS toggle.
Step 2: Turn on MIPS Claims Reporting
Still in Organization Settings > General, scroll to the MIPS Claims Quality Measures card.
Switch on Enable MIPS Claims Reporting.
You'll see the confirmation message MIPS claims reporting enabled.
If the toggle doesn't stick, an error message appears telling you what's missing in your accepted insurance list. See Troubleshooting below.
What Barti does once MIPS claims reporting is on
Barti supports two claims-based quality measures. Both are evaluated only for patients whose primary medical insurance is linked to an accepted insurance with a Claim Type of MEDICARE.
Measure #226 — Tobacco Use: Screening and Cessation Intervention
Barti reads the Tobacco entry in the exam's Social History component and adds the matching G-code to the claim:

Measure #141 — Primary Open-Angle Glaucoma (POAG): IOP Reduction
Barti evaluates the exam's POAG diagnosis, intraocular pressure (IOP) readings, and documented plan, then adds one of M1222, M1223, M1224, M1225, or M1226 to the claim. The code selected depends on whether a prior POAG baseline IOP exists, whether the current exam has an IOP reading, whether a 20% or greater reduction from baseline was achieved, and whether a plan of care is documented for the POAG diagnosis.
You don't add or pick these codes yourself. Barti applies them when the claim is created.
Documentation reminders during the exam
When an exam qualifies for a measure but the required documentation is missing, Barti shows a warning notification while the provider is in the exam:
MIPS QM - document Tobacco Use — the exam qualifies for Measure #226 but no tobacco status is recorded in Social History.
MIPS QM - document IOP — the exam qualifies for Measure #141 but no IOP reading is recorded.
The reminder clears on its own as soon as the provider documents the missing item. Staff can also dismiss it with the X.
What you'll see when it works
Every Medical accepted insurance shows a value in the Claim Type column, with at least one set to MEDICARE.
Enable MIPS Claims Reporting is switched on in Organization Settings > General.
On a qualifying Medicare claim, the procedure lines include the quality codes, and an informational notification reads MIPS QM #226 codes auto-applied or MIPS QM #141 codes auto-applied.
During qualifying exams with missing documentation, providers see the MIPS QM - document Tobacco Use or MIPS QM - document IOP reminder.
Troubleshooting
Why can't I turn on MIPS claims reporting?
Barti blocks the toggle until your accepted insurance list is set up correctly, and shows one of two messages:
"To activate MIPS claims reporting, every MEDICAL accepted insurance must have a Claim Insurance Type set." — One or more Medical accepted insurances has a blank Claim Type. Go through your Accepted Insurances list and set a Claim Type on every Medical row.
"To activate MIPS claims reporting, your accepted insurance list must include at least one MEDICARE insurance policy." — None of your Medical accepted insurances has a Claim Type of MEDICARE. Add your Medicare payer, or correct the Claim Type on the existing row.
Why am I being asked for a Claim Type when I save an accepted insurance?
Once MIPS claims reporting is on, Claim Type becomes required for any accepted insurance with an Insurance Type of Medical. If you leave it blank you'll see "Ensure your accepted insurance list is configured with defined Claim Insurance Type" and the save is rejected. Vision insurances are never required to have a Claim Type.
Why didn't MIPS codes get added to this claim?
Barti only adds quality codes when all of the following are true:
MIPS claims reporting is turned on for your organization.
The patient's primary medical insurance is linked to an accepted insurance with a Claim Type of MEDICARE.
The exam meets that measure's eligibility criteria — for Measure #226 a qualifying encounter code and a patient age of 12 or older; for Measure #141 a POAG diagnosis, a qualifying eye exam or office visit code, and an IOP component on the exam template.
If a patient has Medicare but their insurance record isn't linked to your Medicare accepted insurance, the claim won't qualify. Check the patient's insurance record.
Can I still add quality codes manually?
Yes. Nothing stops you from adding G-codes or M-codes to a claim yourself — useful if you report traditional MIPS measures beyond #141 and #226. Barti's automation covers those two measures only.
Does turning this on change claims that were already created?
No. Codes are applied when a claim is created. Claims created before you turned the setting on aren't retroactively updated.
What happens if I turn it off?
Barti stops adding quality codes to new claims and stops showing the exam reminders. Your Claim Type values stay in place, and existing claims aren't changed. You'll see the confirmation MIPS claims reporting disabled.
